Regarding: Combating SIM Theft.
I can’t believe that this hasn’t been done already (phone accounts get hi-jacked). Setting up a pass phrase, so that if I ever need a new SIM, I have to give the pass phrase before I can activate said SIM.

The basic idea is great. However there must be ways around if you don't have email or access to it. The problem with security is that though you need to make it strong the intention is not possible for a small number so alternative.means must be available.
